1) In this JAMA paper, "Of the 1173 participants (median age, 74 years experienced greater than or equal to 2 conditions by the end of follow-up (316 of 385 [82%], 327 of 385 [85%], and 350 of 403 [87%], respectively, among lifestyle, metformin, and placebo groups). The risk of multimorbidity was lower among lifestyle compared with placebo participants after adjustment for relevant covariates. There was no difference between participants in the metformin and placebo groups. These relationships persisted when diabetes was excluded from the multimorbidity definition. When restricted to dyads of the costliest conditions, the association with lifestyle vs placebo yielded an HR of 0.57. Conclusions and Relevance, among adults with prediabetes at baseline, lifestyle intervention, but not metformin, was associated with a lower burden of multimorbidity. Lifestyle programs may persistently lower the development of chronic conditions. "(Salive et. al. 2026)
The findings from Salive and colleagues reinforce a lesson medicine continues to relearn: downstream therapies rarely overcome persistent upstream dysfunction. GLP-1 receptor agonists are a remarkable advance for improving glycemia, reducing weight, and lowering cardiovascular risk while they are being used. But they do not repeal the laws of biology. If the dietary environment that created insulin resistance, excess adiposity, hepatic steatosis, and chronic inflammation remains unchanged, the metabolic forces driving disease continue beneath the surface. The body may temporarily respond to pharmacologic appetite suppression, yet ultra-processed foods, excess refined carbohydrates, fructose, disrupted satiety signaling, and poor nutrient quality continue to shape the immune-metabolic landscape. This helps explain why weight regain after discontinuing GLP-1 therapy is common and why many patients remain dependent on long-term treatment. Medications can slow the descent, but they rarely reverse the trajectory if the inputs remain the same. Lasting health requires changing the information we deliver to our biology every day through food, movement, sleep, stress, and social connection. Drugs can buy time; lifestyle changes alter the destination.
